For Programs Seeking Accreditation: 

Step 1.

 

Enrollment Application


(Enroll in self-study, for new programs seeking initial accreditation.)
Re-Enrollment Application

 

(Re-Enroll in self-study, for currently accredited programs seeking re-accreditation.)


Step 2.

 

Verification Visit Request


(Request your program's observation.)

(Place your program's visit On or Off Hold.)

(If required) Deferral Verification Visit Request

(Request your program's deferral observation.)

 

 

For Accredited Programs:  

Steps 3 & 4. Annual Report PDF (Required annually to maintain your accredited status.)
(If required) Self Report PDF (Required as needed to report changes to your program.)
